BackgroundCommunity Advisory Boards (CAB) have become essential organs of involving communities in HIV clinical trials especially in developing countries. However, limited empirical evidence exists on the role of CABs in low and middle-income countries including Tanzania. This study aims at exploring the role of CABs in community-based HIV clinical trials conducted in Tanzania.MethodologyWe adopted a phenomenological approach to purposefully select HIV clinical trial stakeholders. These included CAB members, researchers and Institutional Review Board (IRB) members in Tanzania. We conducted In-depth Interviews (...) (IDIs) with ten participants and three Focus Group Discussions (FGDs) with eighteen participants. The data were thematically analyzed with the aid of MAXQDA software version 20.2.1.ResultsThe findings indicate that at every stage of implementation of a community-based HIV clinical trial, a functioning CAB is important for its success. This importance is based on contextualization of the informed consent process and protocol, managing rumours in the community, weighing trial risks and benefits, sensitizing the community, assisting participant recruitment, tracing and retention. However, being perceived as financial beneficiaries than community representatives emerged as a challenge to CAB members. ConclusionThe study empirically indicates the need for functioning CABs in every stage of implementation of community-based HIV clinical trials. The roles of which are interwoven in serving research goals and protecting the interests of the community and that of trial participants. (shrink)

Background Antimicrobial resistance (AMR) is a global threat to human and livestock health. Although AMR is driven by use of antimicrobials, it is often attributed to “misuse” and “overuse”, particularly for antibiotics. To curb resistance, there has been a global call to embrace new forms of moral personhood that practice “proper” use, including prescription, dispensing and consumption of antimicrobials, especially antibiotics. This paper seeks to reflect on complex questions about how morality has become embedded /embodied in the AMR discourse as (...) presented in the data collected on antimicrobial prescription, dispensing and use in human and livestock health in Tanzania, primarily focusing on antibiotics. Methods This reflection is anchored on Jarrett Zigon’s morality framework that is comprised of three dimensions of discourse; the institutional, public, and embodied dispositions. The data we use within this framework are derived from a qualitative study targeting human and animal health care service providers and community members in northern Tanzania. Data were collected through 28 in-depth interviews and ten focus group discussions and analysed through content analysis after translation and transcription. In addition, a review of the Tanzania’s National Action Plans on antimicrobial resistance was conducted. Results Application of the framework demonstrates points of convergence and divergence in the institutional morality discourse articulated by the Tanzania National Action Plans, the public discourse and the embodied dispositions/ lived experiences of human and animal health care service providers and community members. We demonstrate that AMR is not just associated with “inappropriate” behaviour on the part of drug prescribers, dispensers, and users but also with shortcomings in health systems and service delivery. Conclusion Antibiotic dispensing and use practices that may be associated with the development of AMR should not be viewed in isolation from the broader health context within which they occur. Mental fictionalism offers a new approach to these timely questions. Its central idea is that mental states (thoughts, beliefs, desires) are useful fictions. When we talk about mental states, we should be seen as merely speaking “as if” humans (and perhaps other creatures or even artifacts) had such states, in order to make (...) sense of their behavior. -/- This book is unique in presenting and defending three versions of mental fictionalism in a single volume (prefix-semantical mental fictionalism, pretense mental fictionalism, and affective storyism). The authors are three of the world’s most prominent proponents (respectively) of each variety: T. Parent (Nazarbayev University), Adam Toon (University of Exeter), and Tamás Demeter (Corvinus University of Budapest). The book essentially pits these different varieties of mental fictionalism against each other, allowing the reader to size them up. This essay is about the relationship between the notions of practical reason, the will, and choice in Kant’s practical philosophy. Although Kant explicitly identifies practical reason and the will, many interpreters argue that he cannot really mean it on the grounds that unless they are distinct, irrational and, especially, immoral action is impossible. Other readers affirm his identification but distinguish the will from choice on the same basis. We argue that proper attention to Kant’s conception of practical reason as a (...) capacity reveals that these distinctions are neither textually grounded nor philosophically necessary. His moral psychology concerns a single capacity, practical reason, which is the will, and whose actualities in this or that individual fall under the title of choice. Practical reason is the will and choice because it is the rational faculty of desire: a rational being’s capacity to be, by means of her representations, the cause of the actuality of their objects. This, we argue, is entailed by his conception of rational action: action not just in accordance with, but in and through the representation of, principles. The possibility of irrational action is explained not by a distinction between capacities but by the finitude, and thereby the fallibility, of human reason. (shrink)

Paternalist policies in public health often aim to improve people’s well-being by reducing their options, regulating smoking offering a prime example. The well-being challenge is to show that people really are better off for having their options reduced. The distribution challenge is to show how the policies are justified since they produce losers as well as winners. If we start from these challenges, we can understand the importance of the empirical evidence that a very high proportion of smokers regret smoking. (...) In short, it is important that they regret it and important that the proportion is so high. This paper explains how, philosophically, regret can relate to well-being and it considers some of the strengths and weakness in the empirical research that the explanation brings out. The regret case for regulating smoking is indeed strong, although not as strong as the empirical researchers think. It is much weaker for paternalistic intervention in other public health problems, such as obesity and binge drinking. (shrink)

This paper offers a defense of veritic epistemic consequentialism, addressing its principal critiques. I argue that the core of epistemological value lies in its conduciveness to truth, rendering true beliefs intrinsically valuable. In response to the criticism that this approach may sacrifice individual truths for a greater aggregate and undervalues autonomous inquiry, I emphasize the well-connectedness of beliefs. Each belief's content is a proposition. Propositions are classified as first-order, second-order, third-order, etc., depending on what they are about. Higher-order propositions are (...) about lower-order ones. I assume that if an epistemic agent believes in p, then she tends to believe in higher-order propositions that are about p. These higher-order propositions are more structural beliefs of the agent. If p is false, then falsity may spread over the whole network through higher-order propositions about p. Thus, sacrificing a single belief may bring more damage to the network, which is not acceptable for veritic epistemic consequentialism. Regarding the issue of autonomy, epistemic acts like reflection, inference, etc. can be represented as higher-order propositions. Therefore, veritic epistemic consequentialism values them. (shrink)

Central to Kant's ethical theory is the notion of universality, whose apparently canonical expression is his 'Formula of Universal Law' ('FUL'). In the second section of his Groundwork of the Metaphysics of Morals ('Groundwork II'), the FUL stands between the notion of a categorical imperative and later formulations in terms of the notions of humanity as an end in itself, the autonomy of the rational will, and the kingdom of ends. It is meant to be equivalent both to what precedes (...) it and to what follows it. But it has seemed to Kant's readers that neither of these claims to equivalence is true, so that Groundwork II is a comprehensive argumentative failure. I develop a reading of the basic structure of Groundwork II which acquits it of this charge but which, unlike many extant vindicatory readings, does not abandon its expository development by reading later formulations into the FUL. Crucial to my reading is a distinction between three notions of universality which figure in the Groundwork. One of these is expressed by the FUL, but truly central to Kant's ethics is another, that of the will of all, whose philosophical extraction from common moral cognition is the principal task of Groundwork II. This paper demonstrates the undecidability of a number of logics with quantification over public announcements: arbitrary public announcement logic, group announcement logic, and coalition announcement logic. In APAL we consider the informative consequences of any announcement, in GAL we consider the informative consequences of a group of agents all of which are simultaneously making known announcements. So this is more restrictive than APAL. Finally, CAL is as GAL except that we now quantify over anything the agents not in that group (...) may announce simultaneously as well. The logic CAL therefore has some features of game logic and of ATL. We show that when there are multiple agents in the language, the satisfiability problem is undecidable for APAL, GAL, and CAL. In the single agent case, the satisfiability problem is decidable for all three logics. (shrink)

We characterize, in syntactic terms, the ranges of epimorphisms in an arbitrary class of similar first-order structures. This allows us to strengthen a result of Bacsich, as follows: in any prevariety having at most \ non-logical symbols and an axiomatization requiring at most \ variables, if the epimorphisms into structures with at most \ elements are surjective, then so are all of the epimorphisms. Using these facts, we formulate and prove manageable ‘bridge theorems’, matching the surjectivity of all epimorphisms in (...) the algebraic counterpart of a logic \ with suitable infinitary definability properties of \, while not making the standard but awkward assumption that \ comes furnished with a proper class of variables. It was shown recently that epimorphisms need not be surjective in a variety K of Heyting algebras, but only one counter-example was exhibited in the literature until now. Here, a continuum of such examples is identified, viz. the variety generated by the Rieger-Nishimura lattice, and all of its (locally finite) subvarieties that contain the original counter-example K . It is known that, whenever a variety of Heyting algebras has finite depth, then it has surjective epimorphisms. In contrast, we show that (...) for every integer $n \geq 2$ , the variety of all Heyting algebras of width at most n has a non-surjective epimorphism. Within the so-called Kuznetsov-Gerčiu variety (i.e., the variety generated by finite linear sums of one-generated Heyting algebras), we describe exactly the subvarieties that have surjective epimorphisms. This yields new positive examples, and an alternative proof of epimorphism surjectivity for all varieties of Gödel algebras. The results settle natural questions about Beth-style definability for a range of intermediate logics. (shrink)

Social theory remains puzzled by the relation between practices and structures, or the link between ‘micro’ and ‘macro’. Grand theorists including Giddens and Bourdieu have gained distinction for their writings on these questions, trying to marry insights and concerns of a ‘micro’ sociological nature with traditional ‘macro’ structural questions including inequality, power relations, and social reproduction. These theorists arguably fail, however, in their attempts to move social theory beyond traditional dualisms. Relevant but neglected contributions from ethnomethodology are introduced and compared (...) to the work of Giddens and Bourdieu in an attempt to identify and outline an approach to practices and structures that more reliably avoids problems of dualism and reification, and at the same time offers an understanding of structures and their relations to practices that can be much more closely grounded in empirical studies. An assessment of a protocol devised to guide practitioners thinking of using covert video surveillance. Such surveillance is particularly used to help identify cases of Munchausen's syndrome by proxy. The protocol in question has been written by staff at the Academic Department of Paediatrics, North Staffordshire Hospital, Stoke-on-Trent in association with their local Area Child Protection Committee and has been commended by the Department of Health to others wishing to implement covert video surveillance.

Foucault’s critics have often ignored or misunderstool Foucault’s later work, The History of Sexuality and related texts. Only by careful reading of these texts is it possible to appreciate the maturity of Foucault’s social critism, to distil an implicit social theory from his writings, and to gage the true significance of his contributions. In this paper, The History of Sexuality is first placed in the context of Foucault’s earlier works, then used, along with other texts, to answer the most common (...) and famous critiques of his work. In the process, the contours and virtues of Foucault’s implicit social theory are indicated. Similarities with Nietzsche, Weber and Critical Theory are mentioned, but it is suggested that Foucault is in many ways a unique and uniquely significant thinker. Finally, it is argued that Foucault’s works should not be dismissed, as is all too common, but that his provocative reformation of social theory can and should be received and criticized as such. In a series of articles, I argue for a different relationship between investigators and subjects of clinical research – one that is based on partnership in shared aims. This would require significant behavioural change since any relationship of this nature requires each partner to recognise their duties within it. This third essay examines the duties that would fall on patients in this partnership.

Methods developed in a previous paper are employed to define an exact correspondence between the states of a deterministic cellular automaton in 1+1 dimensions and those of a bosonic quantum field theory. The result may be used to argue that quantum field theories may be much closer related to deterministic automata than what is usually thought possible.

As ethical attitudinists say, ethical statements cannot be strictly true or false, since they express wishes or attitudes, not beliefs. However, the wishes expressed by basic moral judgments about human rights are such that it is a necessary truth that those who know what human beings are have them, and those who do not acknowledge these rights show their lack of a living sense of human reality. The same goes for basic judgments about the rights of animals, and it is (...) blindness about the ontology of animal reality which lies behind the cruelty to animals inherent in vivisection and factory farming. One current source of this blindness may be the physicalist metaphysics which is typical of our day, and which should be sharply distinguished from metaphysical naturalism. (shrink)

In a recent book and journal article, I have developed an account of intellectual transparency as an other-regarding intellectual virtue, and have explored its conceptual relationship to the virtue of honesty. This paper aims to further advance understanding of intellectual transparency by examining some of the ways in which the trait is instrumentally valuable. Specifically, I argue that intellectual transparency tends to enhance its possessor’s close personal relationships, work performance, and civic engagement. On account of their intellectual transparency, the intellectually (...) transparent person is likely to enjoy better quality, more satisfied personal relationships such as romantic relationships and friendships. They are likely to contribute to better work outcomes, especially when working in a team context. And, they are likely to be more civically active and to promote epistemic values of democratic deliberation. (shrink)

Psychiatric abuse, such as we usually associate with practices in the former Soviet Union, is related not to the misuse of psychiatric diagnoses, but to the political power intrinsic to the social role of the psychiatrist in totalitarian and democratic societies alike. Some reflections are offered on the modern, therapeutic state's proclivity to treat adults as patients rather than citizens, disjoin rights from responsibilities, and thus corrupt the language of political-philosophical discourse.

Any dialogue conducted via mutually unintelligible languages constitutes no more than a dialogue of the deaf. Yet intelligibility in dialogue at the most basic linguistic level seems to have provoked little extended discussion in China, even though in practice getting one’s ideas across was plainly a major concern, in the late Ming period (1368-1644). Whilst Buddhists of the period had ceased in any real sense to act as translators of fresh Buddhist materials into Chinese from other languages, we do find (...) an essayist with things to say about translation. This was Hanshan Deqing (1546–1623), a major Buddhist monk of the age. Yet his essays are not readily to be found in his Buddhist writings, but in the preface to his annotations to the Daoist classic Daode jing. It is therefore within the context of dialogue with another rival tradition that his remarks were made, and so they have a particular relevance for those with an interest in the conduct of inter-religious dialogue in the Chinese tradition.
